How to submit a Sharing Request.
Follow these steps to submit your sharing request. Your Member Success advocate is available to help you at any point.
Receive Care
Visit your provider and get the care you need. When you do, present yourself as a self-pay and ask to be billed directly. This simple step unlocks lower rates, expedites the determination process, and helps keep costs fair for your community.
Gather Documents
Collect documents as you receive care, including medical records, receipts, and itemized bills with self-pay adjustments.This helps streamline your share request when you're ready to submit.
Submit a Sharing Request
Submit your Sharing Request as soon as possible using the Sharing Request link. You don’t need every document to get started. Submit what you have and add additional items as they become available. Opening your Sharing Request early helps the process begin sooner.
Determination Review
Once EverTrust receives your Sharing Request, your documentation is reviewed under the Member Guidelines. EverTrust will evaluate whether the request may be considered for voluntary sharing, including applicable eligibility requirements, limitations, pre-existing-condition criteria, sharing allowances, and available community-sharing resources. Your advocate will keep you informed throughout the review and request additional information needed to keep you on track.
Paying Medical Bills
If a Sharing Request is determined to be eligible for voluntary sharing under the Member Guidelines, and community-sharing resources are available, EverTrust may facilitate distribution of shared funds in accordance with the approved request. Your advocate will help you understand the process and available options.
What you will need.
EverTrust may request certain documentation depending on your situation. Start gathering these as soon as you can.
Your original itemized medical bills
Full itemized bills from your provider, not summary statements with self-pay adjustments applied. These need to show all procedures, dates, and costs.
Your provider's clinical notes
Medical notes from your provider regarding your visit, diagnosis, and details on the care you received.
Proof of your MRA payment
Receipts showing any payments made toward your Member Responsibility Amount. Your MRA is paid directly to your provider, not to EverTrust. Your advocate can help guide you on how and when to pay if you need it.
Any additional supporting documentation
EverTrust may request additional records specific to your situation. Your advocacy team will guide you on exactly what is needed.
Important things to know before you submit.
For planned procedures, let us know.
Inform EverTrust as soon as a planned procedure is recommended. As a direct-pay patient, it is common for providers to ask for prepayment, that is something we actively help facilitate. The earlier EverTrust is informed, the more it may be able to help you review the Member Guidelines, explore cost-conscious options, obtain pricing information, and understand the documentation that may be needed for a Sharing Request. Early notice does not guarantee that any expense will be eligible for voluntary sharing.
In an emergency, get care first.
When it is a true emergency, do not wait. Get the care you need right away and contact your advocate as soon as you reasonably can after. If a provider asks for your insurance card, let them know you are a self-pay patient. Emergency care does not require prior notification to EverTrust. After receiving necessary emergency care, you may contact EverTrust when reasonably able for information about submitting a Sharing Request. Any request remains subject to review under the Member Guidelines.
With maternity, notify us when confirmed.
As soon as your pregnancy is confirmed, submit a Maternity Sharing Request along with documentation confirming your due date. Your maternity advocate can provide general information and help you understand the Member Guidelines, anticipated documentation, healthcare-navigation considerations, and the Sharing Request process for maternity-related expenses. The sooner we know, the more we can do.
Getting preventive care services, no MRA.
Certain preventive services may be considered under separate preventive-sharing provisions. Whether a particular service is eligible, and whether a Member Responsibility Amount (MRA) applies, depends on the applicable Member Guidelines and the specific facts of the request.

EverTrust Health Share is a nonprofit organization advancing healthcare affordability, cost transparency, patient advocacy, and community education. Free educational resources, general advocacy support, and need-based assistance are available to everyone, with or without membership. Individuals may also choose to participate in a voluntary medical-expense-sharing program governed by the Member Guidelines. EverTrust is not insurance, and no medical expense sharing is guaranteed.
