How to submit a Sharing Request.

Follow these steps to submit your sharing request. Your Member Success advocate is available to help you at any point.

Receive Care

Visit your provider and get the care you need. When you do, present yourself as a self-pay and ask to be billed directly. This simple step unlocks lower rates, expedites the determination process, and helps keep costs fair for your community.

Make sure to — Ask your provider for an itemized bill, not just a summary. Itemized bills are required when you submit your Sharing Request and make the review process much smoother.

Planned & Pre-PaymentFor non-emergent procedures and preventive services, EverTrust can help facilitate pre-payment when appropriate. Submit a Sharing Request early to get your Member Success advocate involved so they can support you before care is received.

Gather Documents

Collect documents as you receive care, including medical records, receipts, and itemized bills with self-pay adjustments.This helps streamline your share request when you're ready to submit.

What to get — Original itemized bills, provider clinical notes, and any receipts showing payment toward your MRA. Your Member Success advocate can help you confirm what is needed for your specific situation.

Submit a Sharing Request

Submit your Sharing Request as soon as possible using the Sharing Request link. You don’t need every document to get started. Submit what you have and add additional items as they become available. Opening your Sharing Request early helps the process begin sooner.

Timing matters — Submit your Sharing Request as soon as reasonably possible. Do not wait for all your bills to arrive. Start early, submit documents as received, and let your Member Success advocate help you stay on track.

Member Responsiblity Amount (MRA) PaymentsYou pay your MRA directly to your provider. If you are unsure when or where to pay, your Member Success Team is here to guide you. As you pay, save your itemized receipts and include them when you submit your Sharing Request. Preventive Sharing Requests are the exception — no MRA required.

Determination Review

Once EverTrust receives your Sharing Request, your documentation is reviewed in accordance with the Member Guidelines. Your submission will be evaluated for eligibility, applicable limitations, pre-existing criteria, and sharing allowances. Your advocate will keep you informed throughout the review and request additional information needed to keep you on track.

Expedite the process — The more complete your submission, the faster your review. Submitting medical records alongside your bills is strongly recommended.

Gathering Medical Notes Read the full guide Think of medical records as your medical notes — they confirm what care you received and why. The fastest way to get them is through your provider's patient portal. No portal? Call or email your provider's office and request them directly. Your Member Success Team is happy to help if you get stuck, just know that delays can happen when records need to go through us, so getting them on your end first keeps things moving.

Paying Medical Bills

When your Sharing Request is approved, the community's shared funds are distributed to you so you can pay your medical bills directly. Your advocate will walk alongside you through this step, helping you navigate the best approach for your situation.

We work fast — When everything is in order, funds can be distributed to you in as little as one business day. We can only move as quickly as the information we have. Timelines and the method of payment can vary depending on the Sharing Request type, provider circumstances, and your situation and your advocate is here to help you navigate that every step of the way.
i How to speed things up?
Communication from EverTrustWe move quickly, and your responsiveness helps us do that. When we reach out with a question or request, a timely reply from you keeps your Sharing Request on track. Funds are typically delivered through an instant virtual debit card sent right to your email — no waiting for a check in the mail. If a reimbursement is needed instead, we will collect your ACH information to make that happen. Your advocate is with you through every part of this.

EverTrust may request certain documentation depending on your situation. Start gathering these as soon as you can.

Your original itemized medical bills

Full itemized bills from your provider, not summary statements with self-pay adjustments applied. These need to show all procedures, dates, and costs.

Your provider's clinical notes

Medical notes from your provider regarding your visit, diagnosis, and details on the care you received.

Proof of your MRA payment

Receipts showing any payments made toward your Member Responsibility Amount. Your MRA is paid directly to your provider, not to EverTrust. Your advocate can help guide you on how and when to pay if you need it.

Any additional supporting documentation

EverTrust may request additional records specific to your situation. Your advocacy team will guide you on exactly what is needed.

For planned procedures, let us know.

Inform EverTrust as soon as a planned procedure is recommended. As a direct-pay patient, it is common for providers to ask for prepayment, that is something we actively help facilitate. The earlier we are involved, the more we can do to confirm your eligibility, explore cost options, and make the process as smooth as possible for you.

i How to lower your MRA
Planned procedure tooltipMembers who take steps to work with EverTrust to find fair-cost procedures may be eligible to lower a portion or all of their MRA. This includes using self-pay resources, comparing providers, or choosing a fair-cost facility when available.

In an emergency, get care first.

When it is a true emergency, do not wait. Get the care you need right away and contact your advocate as soon as you reasonably can after. If a provider asks for your insurance card, let them know you are a self-pay patient. Emergency care does not require prior notification or approval from EverTrust. We will be ready to help you open a Sharing Request once you are able.

i How to lower your MRA
Emergency tooltipYou may be able to lower your MRA by working with EverTrust. When you take steps to reduce your medical costs — such as requesting a self-pay application, gathering itemized bills with self-pay adjustments applied, or participating in attorney-led negotiations — EverTrust may approve a reduction to your MRA. Every effort you make toward lowering costs is an effort that can work in your favor.

With maternity, notify us when confirmed.

As soon as your pregnancy is confirmed, submit a Maternity Sharing Request along with documentation confirming your due date. Your maternity advocate will walk you through everything, from prenatal visits to delivery and postnatal care. The sooner we know, the more we can do.

Maternity tooltipNotifying EverTrust early allows time to plan for fair-cost maternity care. When total eligible maternity costs are kept under $10,000, members may qualify for a $1,500 reduction to their MRA. Using self-pay resources and planning ahead can help support this goal.

Getting preventive care services, no MRA.

Preventive Sharing Requests work a little differently. Memberships can vary on what is considered preventive and which services do not require a Member Responsibility Amount (MRA). Review your membership details or contact your advocacy team for guidance.

Preventive tooltipPreventive services do not require a Member Responsibility Amount (MRA). Some wellness services, such as $0 labs, are available through your Virtual DPC. If a bill is received in advance, prepayment may be available; however, reimbursement is most commonly used. Submissions typically require a receipt, itemized bill, and the member’s ACH information.

What is a Member Responsibility Amount (MRA) and how does it work for me?

Your MRA is the amount you pay toward eligible medical expenses before they become shareable by your community, similar to a deductible. Once you have satisfied your MRA, eligible expenses above that amount are shared. Learn more
How do I handle billing before a scheduled procedure?

For scheduled procedure, providers often require payment in advance, so it's important to plan ahead. Request a Good Faith Estimate from both the facility & surgeon, as these are typically separate bills. Once received, EverTrust can help you coordinate prepayment directly with each provider. What is a virtual debit card?

It's a digital card that is emailed to you and can be used to pay your providers directly. It is issued by EverTrust with a set spending limit based on youre eligible expenses and is the preferred method to help you convinently pay your medical bills.Learn more
Why should I use a surgical center over a hospital for my surgery?

Surgical centers are typically much less expensive than hospitals for the same procedures. They are more specialized and focused on specific procedures, and unlike hospitals, they do not treat a wide range of illnesses at the same time. They are also more accustomed to working with self-pay patients, making pricing, scheduling, and payment arrangements simpler and more transparent. Learn more
